The Request Form
The request form on the Contact page asks only for your name, contact details, and a brief reason for reaching out. Submitted requests are sent through the website to the office's inquiry service so the office can respond. Please do not include clinical details in a web form, email, or voicemail.
Technical and Visit Information
This website receives ordinary technical information when pages are requested, such as IP address, browser type, device details, the page visited, and the time of the request. The hosting platform may use this information to deliver pages, keep the site secure, and produce aggregate visit statistics.
The practice has not added advertising pixels or third-party ad trackers to this website.
